Terms and Conditions for the Sunday Mirror survey draw above.

The prize draw is open to all past and present workers in the NHS in England. You must be over 16 to take part.

The survey prize fund is being provided directly to the winner by the Sunday Mirror.

Incomplete or illegible entries will be discarded.

The winner will be chosen at random from the participants who have indicated that they wish to take part and have completed the survey and contact details correctly.

We reserve the right to re-allocate the prize if we are unable to successfully contact the winner after 7 days of the draw being made. Please ensure your contact details are entered correctly.

The judges decision will be final and no correspondence will be entered into.

Multiple submisions are not allowed and will disqualify you from the draw.
